PATIENT-PROVIDER RELATIONSHIP
Using this website, submitting an appointment request, or sending a message does not by itself establish a patient-provider relationship. A clinical relationship is established only after appropriate registration, consent, evaluation, and acceptance by an Endura healthcare professional.
APPOINTMENTS AND SERVICES
Appointment requests are subject to availability and confirmation. Requested therapies are subject to clinical screening, provider evaluation, contraindication review, and professional judgment. Submission of a form or selection of a service does not guarantee that a particular treatment will be provided.
Applicable pricing, payment requirements, cancellation terms, and service-specific instructions may be presented during scheduling, registration, or treatment.
TEXT MESSAGE TERMS
By voluntarily providing your mobile number and consenting to receive text messages, you authorize Endura Wellness Collaborative to send messages concerning appointments, scheduling, patient communications, care-related information, billing, and requested service updates.
Message frequency varies. Message and data rates may apply. Reply STOP to unsubscribe at any time. Reply HELP for assistance or contact 724-634-8309. Consent to receive text messages is not a condition of purchasing any service.
After you reply STOP, you may receive one final message confirming that you have been unsubscribed. Carriers are not liable for delayed or undelivered messages. Delivery is subject to carrier availability and network conditions.
Mobile numbers and SMS opt-in information will not be sold, rented, or shared with third parties or affiliates for marketing or promotional purposes. Information may be provided to technical service providers only as necessary to operate the messaging service or as required by law.
